Guys who gig with just a 1x12 or horizontal 2x12:

Do you elevate and/or angle the cab? Do you just trust the monitors? I gigged for a long time with a 2x12, but it was diagonal and frankly huge, only a few inches shorter than a 4x12, so I never really ran into that problem.
"If there was only a way that I couldn't even any less than I already can't then I'd gladly not, but I think I've entered some quantum paradox, dark matter levels of can't evening where the total absolute value of not evenness exceeds all ability to explain the can'ting." - MR RUBATO

milk crates are an economical way to do this and they also double as storage devices

I gigged with a Mesa Recto 2x12 and I never had a problem hearing myself. I had v30's in there, which might seems kind of directional, but it was never an issue.

I lean my 1x12 cab back with one of those little folding stands. It has two benefits in that it points it towards my head and decouples it from the floor to get rid of that booming quality you get with some venues.

I always raise my cab to ear height, be it 212 or 412. No point in blasting my ankles with volume.
